I think it might be 3. Have yet to see 4 yet. Have gotten 3 QA on Herc hands, and 1 & 2 on valor pieces. Never got or seen 4 from me or anyone else.
Just 3. I'm looking for the Dev post that made it look like all our Reisenjima armor would soon be Thaumus Coats... I don't remember where that was posted.
QA has a different cap than TA or DA, if I remember correctly. And it is possible to get all three on a piece, per that Dev post. I don't think we've seen that yet, so the chance is very, very small (obviously). Found it.
