Make is whoever makes it (Dell, HP, Sony, etc) model should be located on the label that has the serial number on it.

There are literally countless different types of fans out there though... You'd be pretty lucky to just pick one randomly that looks 'kinda like' yours and have it all fit and work like it's supposed to.
If it were a desktop, I could probably tell you in about 30 seconds what kinda fan it is, because they're all pretty basic, but I've never worked on laptops and have no experience with the fans that are used. Edit: Meant to post this was always my favorite card, although she is a little fat, I won many a duel with my Beaver warrior!
